Marriage forms

The forms on this page are for official use under the Marriage Act 1961 and Marriage Regulations 2017.

Do not change these forms or they may become invalid.

Buy official forms from CanPrint

You can download all forms except the Form 15 Certificate of Marriage.

You must buy this form from CanPrint Communications which is the approved supplier. They can only supply the form to registered marriage celebrants.

You must give them your celebrant registration number anytime you buy marriage stationery.

You can buy forms that are not the Form 15 from other suppliers. But they must meet the requirements under the Marriage Act. If they don’t, they may be rejected by the registry of births, deaths and marriages.

Under section 50(3) of the Marriage Act 1961, this form shall be printed on the reverse of 1 of the official certificates of marriage.

Under section 50(3) of the Marriage Act 1961, 1 of the official certificates of marriage shall be on the reverse side of the paper with the declaration of no legal impediment to marriage.

Approved marriage celebrants

Under Paragraph 73(5) of the Marriage Regulations 2017, an authorised marriage celebrant must keep a record for each Form 15 marriage certificate they use.
